Step 1: Submit a Chicago 311 Service Request
If you haven’t already, visit Chicago 311 Service Request and follow the prompts to report the issue. Your 311 Service Request (SR) is the first step in the process.

Step 2: Submit the issue to EGA
Once you have a 311 SR, submit the issue through this form. EGA will track outstanding requests, coordinate follow-up with the 48th Ward Office and appropriate City departments, and group and prioritize similar concerns across the neighborhood.
